CASE MANAGERS

Case Manager/Counselor needed for an all women's treatment center in Costa Mesa. Candidate would be facilitating individual counseling sessions, family counseling sessions, and group process. Candidate will work as part of the treatment team already established and will be teaching skills such as life skills, coping skills, anxiety management, self-esteem, and job search skills. Candidate will be responsible for contacting all the providers and the clients families to keep all involved on the same plan, as well as create a discharge plan for all clients. Requirements: CATC1, CADC I/II, LCSW or any other counseling certificate recognized by the state of california. A minimum of two years experience in residential treatment. Experience with state laws and regulations as well as HIPAA regulations. Experience with utilization reviews, insurance policies, and documentation requirements.

The primary goal is to provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, to promote continuity of care through integrating case management, review, and discharge planning.

The goal of each counselor is to provide counseling and educate the clients to the disease of addiction and assist them with:
* Serve as a client advocate, enhancing a collaborative relationship to maximize the clients ability to make informed decisions.
* Facilitate the development of a treatment plan for each client and that all necessary assessments are completed.
* Conduct and facilitate individual, group and family counseling as appropriate for each client and the treatment facility as a whole.
* Facilitates patient care or meetings to review treatment goals and optimize treatment resources Collaborate with all members of clinical staff in the development and execution of the plan of care and achievement of goals.
* Ensure that all clients maintain their sobriety while in treatment through meetings, groups and drug and alcohol testing carried out within the established timeframe and that the results are promptly reviewed. Meet with clients to discuss and coordinate their life skills, emotional growth, coping skills, relapse prevention, anger management, anxiety management, employment issues, self-esteem, and aftercare plans.
* Create and complete psych-socials and treatment planning for each client.
* Maintain official records and documents, and ensure compliance with federal, state and local regulations. Responsible for contacting and calling providers and families weekly as long as there is a signed consent to do so on record.
* Create a discharge plan for each client.
* Be available for on-call every other week.
